Intake Appointments: Information regarding your intake paperwork will be sent to your email address on file. If you do not complete prior to your appointment, please arrive a minimum of 20 minutes prior to your appointment time to fill out necessary paperwork.
Cancellation Policy: We require a 24 hour notice of cancellation. Please note, if you do not show for your intake appointment and do not call to cancel, you will not be rescheduled at our facility.
